What areas do you serve?

Who pays for the services?

Most of our services are funded through Medicaid waiver programs and other state-funded programs. We help families navigate eligibility and connect with the right funding sources.

Yes. We match each individual with a consistent caregiver based on their needs, location, and personality. The same person comes every time, building a real relationship.

How do I make a referral?

You can submit a referral through our website, call us at (952) 256-4240, or email us. Case managers, family members, and social workers can all make referrals. We call back the same day.

What qualifications do your caregivers have?

All caregivers pass thorough background checks, complete required training, and are supervised by qualified professionals. Many have years of experience in home care and hold relevant certifications.
