How is the support plan created?
It starts with a detailed assessment where we learn about your loved one's abilities, challenges, preferences, and goals. We work with their care team and family to build a plan that addresses their specific needs while respecting their independence.
Will we have the same caregiver each visit?
We prioritize consistency because relationships matter. You will typically have the same primary caregiver, with a backup caregiver who also knows your loved one in case the primary is unavailable.
Can the level of support change over time?
Yes. Needs change, and our support changes with them. We review the care plan regularly and adjust the level and type of support as needed. Whether your loved one needs more help or is becoming more independent, we adapt.
